Member Cancellation request
CANCELLATION FORM- The form is a request until management signs the form. There is a
$19 cancellation processing fee. The account must be current and out of term in order to cancel with a 31-day written notice. All Cancellations require a 31-day written notice at The Ohio Gym, before the first of each month. The processing fee and any draft with in the 31-day notice must be collected at the time of cancellation.
